Before applying for ethical approval through FERC, all research proposals must have undergone a research proposal defense. A letter of DRP acceptance/approval shall be submit with other application documents.

There will be 4 forms that need to be included for the initial application:

1. Ethics Application Form FERC1

2. Participation Information Sheet FERC2

3. Assent Form FERC 3 (if applicable)

4. Checklist for Submission Form

For those who are doing research that do not require ethical approval, you may fill up the Exemption from Ethical Review Form (FERC4)

Studies that do not require ethical approval are as below:

1. Systematic review study

2. Laboratory study that does not involved human samples

If you have received the ethical approval and there is modifications of your research methodology, you are required to inform ERC by filling up the Post Approval Amendment Form (FERC 5)

The submission of the application form and documents should be emailed to the FERC email via: ercdentistry@gmail.com

Upon submission, all applications will undergo an initial administrative screening to ensure completeness and compliance with the required documentation. Complete applications will then be reviewed by appointed reviewers according to the level of risk involved in the study. The review process is conducted in accordance with established ethical guidelines to ensure the protection, safety, and rights of research participants.

Following the review, applicants will be informed of the outcome via email. The review outcome may include:

1. Reviewed: Minor Amendment Required

2. Reviewed: Major Amendment Required

Applicants are required to address all reviewer comments before final approval can be granted. The corrected application will be reviewed again by the reviewer upon re-submission. Once the application is deemed satisfactory by the reviewer, the result will be discussed and endorsed in the monthly ERC meeting. (FERC Meeting Schedule)

Applications classified as More than Minimal Risk (MMR) will be forwarded to the Research Ethics Committee (REC) for further review and approval.

Applicants will be informed of the status of their application via email at least two (2) weeks after submission. The application status will be one of the following:

Under Review

Reviewed: Minor Amendments Required

Reviewed: Major Amendments Required

Approved

Forwarded to the Research Ethics Committee (REC)

If applicants do not receive any email notification regarding the status of their application, they are advised to contact the secretariat at: ercdentistry@uitm.edu.my

Any amendment to the research protocol needs to be informed to the FERC. Applicant are required to fill up the FERC 5 Form and email to the secretariat at: ercdentistry@gmail.com

The process of the post approval amendment will be the same as the one describe in Review by FERC Panel Section.

Upon completion of the research, you are required to submit your final report using Form FERC 8.
